We secure this on-line enrollment form with Digicert Inc. All transactions sent through our website are safe so that only you and Independent Health will be able to view the data. You may confirm the security by visiting http://www.digicert.com.
3. Is this enrollment official?
Yes. By completing and submitting this enrollment form you are sending an actual enrollment election to Independent Health.
